We partnered with Krames, a nationally recognized expert in patient education materials, to create an Essure Confirmation Test brochure that gives patients an overview of the Essure Confirmation Test test following the Essure procedure: “After the Essure procedure—HSG (hysterosalpingogram).” We would like to thank Drs. CC-1066 Essure Confirmation Test Letter Missed Appointment: This letter can be sent to women who have missed their Essure Confirmation Test appointment, to remind them that they need to reschedule their appointment.
CC-1066 Essure Confirmation Test Letter Reminder:This letter can be sent to patients who have already scheduled an Essure Confirmation Test appointment, to remind them of their appointment date, time, and location.

We suggest that you send the above Essure Confirmation Test letters with return receipt and document any phone conversations your office has with the patient regarding her Essure Confirmation Test. These letters may provide proof of notification in the unfortunate event that a patient does not comply with the Essure Confirmation Test requirement and becomes pregnant. Check with your legal counsel on the use of these letters with regards to liability.
